Probably sounds kinda dumb and a bit pointless as it's more of a fluff thing, but what about a small thing which on a player's profile, depending on if they're a hero or villain (for an example), highlighting that.
So say I got character A, they're a hero, below their main description, injuries, stats or w/e, they get a star along with a number next to it.
Whereas for character B, if they're a villain, they get a either a red skull or something next to it, along with the number next to it.
Now you might be thinking; what's the point of this? Haven't you got rogues, scratched headbands? Ect. ect.
Yes, true, but what about people who aren't rogues? What if you got a prominent Kage, but you want a way to know aside from events and world posts, on how the world sees this Kage from a glance. With this system, you can get an idea; it can be updated at each LC at an admin's discretion or maybe after the conclusion of an event, if it's impacting the world or a village or some group of people. It'd be a nice way to provide consequence outside from events and player interaction, on more of a meta-system level.
Obviously, this system should have a toggle, incase people want to keep it hidden for w/e reason.
Mainly inspired from Fable 2 and Oblivion mostly.

Honestly I'd say we should just add an actively updated notable shinobi list. Unlike the bingo book this would be used for both sides of the coin. It could be relatively simple: Name, alias, rank, and what they were put on the list for (events, abilities, etc). It'd actively simulate a living world in which rumors and such circulate about and be another small thing for players to shoot for.
